We will be evaluating the use of this app to collect data to help understand the use of new technologies for the distribution of health-related information in the event of a public health emergency. As part of this evaluation, after you have downloaded and used the app for a while, we will be providing a few short questions through the app, to learn your thoughts about this app. Continued use of this app and response to these questions is voluntary; you can choose to uninstall this app at any time, or refuse to respond to these questions if you would like to. These questions will take about a minute or two to complete.
We will not be using any personal information as part of this evaluation (including personal identifying information, date of birth, or IP addresses. However, if you decide to create a profile to utilize the personal immunization tracker, we will request information on date of birth information and vaccine records. This information will be stored in a secured website and will not be used for research purposes without your consent.
The new version of ReadyVax (2.0) includes an immunization tracker in the patient role. This tool allows the user to create a profile and enter vaccine information and date of birth to produce personalized recommendations. This information will be stored in a secured website and will be used by the program to personalize vaccine recommendations based on your age. These recommendations are based on the CDC Recommended Immunization Schedules for Children and Adults 2017. The immunization tracker uses the standard schedule and does not account for specific conditions or circumstances that may require different vaccination schedules (such as- but not limited to- immunodeficiency, HIV, high-risk of disease, or international traveling). This tool should not replace medical advice. Creation of this user profile is optional. Not creating this profile will not affect access to the other sections of the app.
